New Ordering Process

Thanks for all you do for your testing program! As of October 31st, we’ve updated our ordering process. This form is no longer in use. To order supplies, log into your account at, and follow the steps outlined in this article.


Only Test Champions can order supplies through the updated method. To see your access, go to Switch Portal Roles. If you do not have that access, please contact your organization’s Test Champion who will be able to place an order. 

If you placed an order before October 31st and it has not yet arrived, this update has no impact on delivery times of those orders.
